Now Hiring Family Nurse Practitioner for PCHCs School-Based ClinicMount Pleasant High School in Providence, RI A primary care clinic inside the high school for the students and their families School hours and School-calendar year position (with an option for year-round work)Providence Community Health Centers supports schools located in Providence, RI through the SMART school-based clinics.
We are hiring Family Nurse Practitioners to help us improve the health, education, and wellness of our urban youth.
Qualifications:
Family Nurse Practitioner certification, licensed in Rhode Island Minimum 3
years of FNP experience in primary care Desire to care for students and their families in a school-based clinic Interest in working the school calendar year and school hours Serving as an extension of the students primary care team and working in the Providence schools, our FNPs support students, school staff and students families with physicals, managing chronic conditions, injuries, and other primary care needs.
Each FNP in our school-based clinics collaborates with a behavioral health clinician and RN.
Currently, the PCHC Smart team serves one elementary school, one middle school, and one high school in the city of Providence.

We achieve this by identifying and delivering interventions to address the physical, social, emotional and behavioral barriers to classroom success, and thus creating optimal conditions for learning through the deployment of Active Access to deliver Active Care for all students, school faculty and staff, and students families.
