Hospice Certified Nursing Assistant (CNA) - $22-23/Hour - Providence, RI

Certified Nursing Assistant (CNA), Hospice You Will (Responsibilities) Demonstrate competency in delivering comprehensive and personalized care to patients and families including (but not limited to) the following Assisting patients with the completion of activities of daily living (ADLs) Monitoring vital signs - reporting findings to the appropriate nursing personnel Promoting dignity and comfort at the end of life Perform patient care as assigned, and collaborate with a multidisciplinary care team to deliver care to clients both at home and in the community.
Provide care to patients and families as outlined within the plan of care in a variety of settings including but not limited to patients residence, assisted living facilities, and skilled nursing facilities.
Maintain continuity of patient care by communicating information timely and effectively with all team members providing care to the patient.
Complete documentation in a timely and accurate fashion, and at the point of care, according to industry standards, conventions and guidelines Maintain the comfort, privacy, and dignity of each patient in the delivery of services to them.
Fully understand all aspects of patients rights, including the right to be free of restraints and free of abuse.
Responsible for promptly reporting to the clinical office or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
Assist in maintaining a safe, neat, and clean environment Observe patients for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel.
Communicate and interact effectively and tactfully with the patient, visitors, families, peers, and supervisors.
Adheres to all company policies, procedures, and standards of conduct.
Performs additional duties as assigned.
